Work Environment

Key Responsibilities

• Oversee and direct the implementation of individualized treatment plans aimed at preventing disability, alleviating pain, and restoring muscle function in accordance with medical guidelines.
• Conduct thorough evaluations of clients' physical abilities through diagnostic assessments, enabling the development of tailored rehabilitation strategies.
• Assess and modify treatment plans based on client progress, communicating effectively with physicians and case managers.
• Deliver physical therapy services in the home setting as prescribed, ensuring the highest standards of care and implementing recommended home modifications to enhance patient independence.
• Administer therapeutic exercises and treatments, including traction and massage, while providing clear instructions and motivation to clients to assist in their recovery journey.
• Participate actively in interdisciplinary team meetings and case conferences to contribute to holistic patient care efforts.
• Document all clinical records meticulously, including patient evaluations, care plans, and progress notes, ensuring compliance with organizational and legal standards.
• Train and guide Physical Therapy Assistants in providing care, helping them develop their skills and ensure consistency across treatment modalities.
• Stay accessible for consultation during the treatment provided by assistants, ensuring a collaborative approach to care.

Patient Population / Client Focus

You will primarily work with orthopedic patients, addressing a variety of issues related to musculoskeletal concerns, including post-operative recovery, injury rehabilitation, and chronic pain management. Qualifications

• A Bachelor's Degree in Physical Therapy from an accredited program is required.
• Current licensure as a Physical Therapist in New York, along with a CPR certification.
• At least one year of professional experience in a similar role focused on orthopedic care.
• A valid driver's license and automobile insurance, enabling you to provide home care services effectively.
