Key Responsibilities\n

• Act as a primary liaison for clients, families, and outpatient providers, ensuring seamless communication and understanding of treatment plans and progress.

• Engage with insurance companies for pre-certification and utilization review, helping clients access necessary resources without unnecessary barriers.

• Participate in discharge and aftercare planning, collaborating with multidisciplinary teams to formulate comprehensive care strategies that support client transitions.

• Offer therapeutic meal support sessions by modeling healthy relationships with food, addressing food-related issues, and promoting positive coping mechanisms.

• Deliver safe, empathetic, and affirming care tailored to each client's unique circumstances, recognizing the importance of their intersecting identities and life experiences.

Patient Population / Client Focus\n

Our client focus includes individuals of all ages, with an emphasis on older adults who may face unique challenges related to mental health. You will be instrumental in addressing complex emotional issues, ranging from isolation and grief to chronic illness management. Qualifications\n

• A Master's degree in Social Work from an accredited institution.

• A valid Licensed Clinical Social Worker (LCSW) license in the state of Oregon.

• Extensive experience working with older adult populations, demonstrating sensitivity to their specific mental health needs.

• Strong clinical assessment and diagnostic skills, with the ability to create and implement effective treatment plans tailored to individual client needs.

Preferred Experience\n

While all candidates that meet the minimum qualifications will be considered, preference will be given to those with specialized training in geriatric social work, cognitive behavioral therapy (CBT), or trauma-informed care approaches. Experience in navigating healthcare systems and advocating for vulnerable populations will also be advantageous.

Compensation and Benefits\n

The salary for this role ranges from USD 90,000 to 104,000 annually, commensurate with experience and qualifications.
