Diabetes Educator

UCHealth is seeking a Diabetes Educator to provide comprehensive education and support for patients and families managing diabetes within our Healthcare & Medical Services department. In this role, you will assess learning needs, develop individualized care and education plans, deliver group and one-on-one teaching, and coordinate with an interdisciplinary team to optimize glycemic control and prevent complications. You will use evidence-based guidelines, EMR documentation, and diabetes technologies while contributing to quality improvement.
