Work Environment\n

This role is exclusively remote, allowing for flexibility in your work environment while ensuring that our senior clients receive uninterrupted support. You will be embedded within school facilities that serve as community hubs for senior outreach. The majority of your work will be conducted virtually, coupled with necessary field visits to clients’ homes to conduct assessments and provide direct care.

Key Responsibilities\n

• Conduct comprehensive psychosocial assessments to identify the needs of seniors, focusing on emotional, mental, and social factors.

• Collaborate closely with clients and their families to develop and implement personalized care plans aimed at meeting their unique goals and preferences.

• Provide individual counseling and emotional support to seniors, guiding them through significant life transitions such as relocation to assisted living or dealing with loss.

• Advocate for client rights and help them access essential community resources, ensuring they receive the care and services they need.

• Work as part of an interdisciplinary team that includes nurses and healthcare professionals to create a cohesive approach to client care.

• Perform regular home visits to monitor clients’ well-being, evaluate their living conditions, and address any safety concerns that arise.

• Facilitate family meetings to resolve conflicts and enhance communication among family members involved in the clients’ care.

• Educate clients and their families about local resources, support groups, and social programs that can enhance social engagement and improve overall quality of life.

• Maintain accurate and comprehensive documentation within electronic health records (EHR), ensuring compliance with ethical standards and regulatory requirements.

Patient Population / Client Focus\n

This position primarily serves elderly clients, many of whom may experience challenges related to aging, such as mobility issues, cognitive decline, social isolation, and mental health concerns. Qualifications\n

• A Master’s degree in Social Work from an accredited institution.

• Valid LMSW licensure in the state of Florida.

Compensation and Benefits\n
