Desert Blossom Orthopedic and Sports is seeking a dedicated and skilled Physical Therapist to join our team in South River, New Jersey. This full-time position focuses on providing patient-centered physical therapy in a home care setting. The ideal candidate will be responsible for creating individualized treatment plans to restore mobility, alleviate pain, and enhance the overall quality of life for our clients.

Work Environment

As a Physical Therapist in our organization, you will work primarily in the client's home environment, which allows for tailored treatment strategies that consider their specific living conditions. This onsite role emphasizes hands-on therapy and direct interaction with clients, building strong relationships and trust. We operate a Monday through Friday schedule, prioritizing work-life balance while ensuring our patients receive timely and effective care.

Key Responsibilities

• Direct physical therapy treatment to prevent disability, relieve pain, improve muscle function, and maintain maximum performance in alignment with individualized treatment plans.
• Supervise and guide Physical Therapy Assistants in the execution of treatment strategies while ensuring compliance with the established plan of care.
• Conduct comprehensive evaluations of clients' functional levels through diagnostic testing and assessments of muscle, nerve, and joint functions.
• Assess clients' homes for rehabilitation potential and make strategic recommendations to foster independence and safety in their environment.
• Administer therapeutic exercises and treatments including manual therapy, traction, and massage to facilitate recovery and pain relief.
• Evaluate, fit, and adjust prosthetic and orthotic devices, providing recommendations for modifications to enhance client mobility.
• Maintain accurate clinical records, including OASIS evaluations, care plans, progress notes, and discharge summaries to track patient progress and inform ongoing care.
• Engage in interdisciplinary team meetings, contributing to a holistic treatment approach by collaborating with physicians, case managers, and other healthcare professionals.
• Train clients and their families on proper use and maintenance of therapy devices, reinforcing their understanding of the overall therapy program.
• Participate in ongoing professional development to stay current with physical therapy practices and innovations.

Patient Population / Client Focus

The clients you will serve often include individuals recovering from surgeries, injuries, or chronic illnesses requiring tailored physical therapy interventions. In this role, understanding unique client backgrounds, including their motivations and challenges, is vital to designing effective treatment plans that enable improved overall function and independence.

Qualifications

• Applicants must have completed a Doctorate in Physical Therapy (DPT) program and hold an active Physical Therapist license in the state of New Jersey.
• Must possess an impeccable professional record, free from sanctions or disciplinary actions.
• Demonstrated proficiency in a wide range of manual therapy modalities is essential.
• Effective communication and interpersonal skills are crucial for collaborating with clients, families, and interdisciplinary teams.

Preferred Experience

• Previous experience in home health care is preferred, as familiarity with working in clients' homes can enhance the understanding of client-centered care.
• Proficient use of Electronic Medical Record (EMR) systems, particularly Web PT, for accurate documentation is highly desirable.
• A robust knowledge base regarding current physical therapy techniques and research-supported practices is essential for successful treatment outcomes.

Compensation and Benefits
