Key Responsibilities\n

• Communicate effectively with physicians and other healthcare professionals about patient progress, needs, and any emerging issues to ensure integrated care.

• Maintain accurate and timely clinical documentation, including progress notes and treatment plans, following departmental policies and best practices.

• Work collaboratively with a multidisciplinary team to develop individualized treatment plans, discharge strategies, and conduct psychosocial assessments to inform care.

• Provide counseling and crisis intervention services tailored to the unique emotional and behavioral challenges faced by patients.

• Support patients through discharge planning, coordinating care with community providers to facilitate a smooth transition to continued support.

• Engage with families or support systems to keep them informed and involved in the treatment process, promoting a comprehensive support network for the client.

Patient Population / Client Focus\n

In your role as an LPC, you will engage with a diverse client population experiencing a range of mental health challenges, including anxiety disorders, mood disorders, trauma-related issues, and more. This responsibility requires not only therapeutic expertise but also cultural sensitivity to effectively connect and work with clients from different backgrounds and personal experiences.

Qualifications\n

• A Master's degree in Professional Counseling or a closely related field from an accredited program.

• Current licensure as a Licensed Professional Counselor (LPC) or equivalent.

• At least two years of relevant experience in providing mental health direct services.
