Key Responsibilities\n

• Collaborate with primary care providers to identify, treat, and manage the mental health and psychosocial issues of patients.

• Conduct comprehensive patient intakes, focusing on diagnostic evaluations and functional assessments.

• Make informed recommendations to primary care providers regarding patients’ treatment goals and individualized care plans.

• Deliver brief, focused interventions for patients requiring mental health services, ensuring timely and effective support.

• Throughout treatment, initiate follow-up communications to assess patient progress and modify treatment strategies as needed.

• Develop relapse prevention plans and provide ongoing support to help patients maintain stability and functionality.

• Identify at-risk patients and devise intervention plans to prevent deterioration of their mental health.

• Monitor and coordinate the delivery of behavioral healthcare services, including referrals to external treatment facilities as appropriate.

• Document patient progress meticulously, maintaining accurate and current treatment records.
\n
• Actively engage with primary care providers to keep them informed about patients' needs and progress while contributing to collaborative treatment planning.

• Facilitate lifestyle changes among patients with chronic health issues, empowering them to adopt healthier habits.

• Provide educational resources and self-management skills to encourage patient autonomy in their treatment journey.

• Consult with clinic management and team members regarding behavioral services and enhancement of clinical outcomes.

• Assist patients in adhering to medical treatments prescribed by primary care providers, including addressing medication side effects.

• Uphold ethical standards and compliance with applicable regulations in all clinical practices.

• Perform other duties as assigned to support the clinic's mission and objectives.

Qualifications\n

Compensation and Benefits\n

The compensation for this full-time position ranges from USD 87,000 to 105,000 annually, commensurate with experience and qualifications.
