Occupational Therapist (OT) – Home Health & Facility | Ransomville & Niagara Falls, NY | PRN & Part-Time | $50–$75 Per Visit

Stern at Home Therapy is hiring an Occupational Therapist (OT) to provide one-on-one care to patients in their homes and at an established facility in the Ransomville area .

Choose between part-time W-2 employment with benefits or a flexible PRN opportunity based on your availability.

This opportunity is ideal for a licensed Occupational Therapist looking to build a steady part-time caseload or add flexible PRN visits to an existing schedule, treating one patient at a time with support from an experienced administrative and clinical team.

What You'll Do:

As an Occupational Therapist with Stern at Home Therapy, you will evaluate and treat a primarily geriatric patient population throughout the Ransomville and Niagara Falls areas.

Your caseload may include both home health visits and treatments at our established facility in Ransomville , providing variety while minimizing unnecessary travel.

Responsibilities include:

• Perform occupational therapy evaluations and develop individualized plans of care.
• Provide one-on-one treatments based on each patient's functional needs and goals.
• Manage individualized treatment plans with clinical autonomy.
• Help patients improve independence, safety, mobility, and their ability to perform daily activities.
• Complete accurate and timely clinical documentation.
• Communicate with patients, families, physicians, facility staff, and members of the clinical team.
• Coordinate with our administrative team regarding scheduling, patient care, and logistics.
• Manage your caseload based on your availability and patient needs.

Compensation:

Part-Time W-2:

• $50-55 per follow-up visit.
• $65 per 4-unit evaluation.

PRN:

• $62 per 4-unit follow-up visit.
• $75 per evaluation.

Qualifications:

• An active Occupational Therapist (OT) license in New York is required.
• Reliable transportation required.
• Strong clinical, communication, and time-management skills.
• Ability to work independently in home and facility-based settings.
• Home health experience is preferred but not required.
