Key Responsibilities\n

• Conduct comprehensive assessments to determine the mental health needs of patients and develop tailored treatment plans.

• Facilitate effective communication between supervisors, family members, and other healthcare professionals to monitor and discuss individual patient progress.

• Provide timely schedules for appointments, home visits, and observations to ensure consistent monitoring and adaptation of treatment approaches.

• Coordinate home visits, managing logistics for observations, chart reviews, and data analysis to obtain a holistic understanding of patient needs.
\n
• Collaborate in Interdisciplinary Team Meetings to evaluate treatment efficacy and make necessary adjustments to care plans.

• Document and complete monthly and quarterly progress notes to maintain accurate patient records and ensure compliance with healthcare standards.
\n
• Organize and deliver training sessions for staff to ensure that all team members are well-equipped to implement treatment plans effectively.

• Engage in follow-up interviews and ongoing support activities to address the evolving needs of the individuals under your care.
\n
• Maintain meticulous records to ensure that patient information is readily accessible to authorized individuals within the healthcare community.

• Act as a liaison with the public and mental health community, helping to develop resources and programs that align with community health initiatives.

Qualifications\n

• A Master’s or Doctoral degree in Counseling, Psychology, or a related field is required.

• Possession of an active and unrestricted LPC license in the state of Texas.

• At least two years of experience in a counseling role, preferably in a clinical setting.

• Demonstrated expertise in family therapy, couples therapy, substance misuse, or trauma is highly preferred.

• Solid understanding of pertinent legal and ethical standards governing mental health practices.
