Key Responsibilities\n

  Conduct ongoing assessments and evaluations to identify patient needs, developing personalized intervention strategies to ensure effective treatment outcomes.

  Facilitate communication among supervisors, healthcare team members, family members, and other stakeholders to collaboratively monitor an individual’s progress.

  Develop and provide a tentative schedule to supervisors detailing appointments, home visits, and direct observations aimed at tracking client progress and treatment efficacy.

  Coordinate and implement home visits, including in-home observations and chart reviews, to gain comprehensive insights into clients' environments and needs.

• \n

  Ensure the effectiveness of program plans by actively participating in interdisciplinary team meetings, collaborating with peers to assess treatment strategies and outcomes.

  Document monthly and quarterly progress notes, ensuring accurate and accessible records that reflect patient progress and inform future care plans.

• \n

  Lead training initiatives within the team to ensure all staff members are fully competent in implementing program plans and completing necessary documentation.

  Conduct follow-up interviews, home visits, and observations to provide ongoing support tailored to each individual’s unique circumstances.

  Act as a liaison with the broader mental health community, fostering relationships that support program development and enhance service accessibility.

  Assist in developing training procedures and materials aimed at continuously improving service delivery for our diverse clientele.

Patient Population / Client Focus\n

Qualifications\n

  A Master’s or Doctoral degree in Counseling, Psychology, or a related field is required.

  Possession of active and unrestricted licensure as a Professional Counselor (LPC) in the state of Texas is essential.

  A minimum of two years of counseling experience in a clinical setting is required, demonstrating effective treatment methodology.

Compensation and Benefits\n

This full-time position offers a competitive annual salary ranging from USD 90,000 to 100,000, commensurate with experience and caseload availability.
