Key Responsibilities\n

• Conduct thorough assessments of patients, including ongoing evaluations to tailor mental health services to individual needs.

• Facilitate communication among supervisors, clinical staff, family members, and other stakeholders regarding patient progress and treatment plans.

• Develop and submit a tentative schedule of appointments and home visits to supervisors, ensuring timely follow-up of care.

• Coordinate and conduct home visits to gather data through in-home observations, chart reviews, and staff interviews.

• Ensure effective implementation of program plans, contributing to the overall success of treatment modalities.

• Actively participate in Interdisciplinary Team Meetings, sharing insights and collaborating on strategies for patient improvements.

• Document patient progress with monthly and quarterly notes, adhering to all regulatory and ethical guidelines.
\n
• Provide training sessions to ensure the team is equipped to implement program plans effectively and accurately.
\n
• Conduct follow-up interviews and observations to assess ongoing needs, providing additional support where necessary.

• Maintain meticulous records to ensure that information is readily accessible to all relevant parties, promoting transparency and support.

• Serve as a liaison between the clinic and the mental health community, facilitating the development of community resources.

• Assist in enhancing program efficiency by developing training materials and procedures uniquely suited to our clientele's needs.

Qualifications\n
