The TCM Coordinator plays a vital role in facilitating smooth transitions of care for patients following hospital admissions, discharges, and emergency room visits. Reporting to the Manager of Patient Management, this role involves coordinating care plans, conducting follow-ups, and collaborating closely with care teams to ensure comprehensive patient management and quality improvement.

Responsibilities

• Coordinate care for patients post-discharge to ensure continuity and compliance
• Review daily cases for admissions and discharges related to inpatient stays and ER visits
• Conduct post-discharge follow-up calls to review medications and patient status
• Schedule Transitional Care Management (TCM) appointments with Primary Care Providers (PCPs)
• Perform medication reconciliation to ensure accuracy and safety
• Document all patient interactions and follow-ups meticulously
• Assist with quality improvement initiatives within patient management processes
• Collaborate with interdisciplinary care teams to optimize patient outcomes

Preferred Qualifications

• Medical Assistant or related clinical experience
• Understanding of medical terminology
• Minimum of 3 years experience in a clinical setting
• Experience with care coordination and chart reviews
• Strong communication and patient education skills
