Work Environment

This position is primarily onsite, delivering patient care directly within home care settings, which allows for personalized therapy sessions in the familiarity of your patients' homes. Key Responsibilities

• Assess and analyze patients' condition by conducting thorough evaluations and documenting treatment plans with accuracy.
• Deliver tailored physical therapy treatments for patients with orthopedic conditions, ensuring adherence to evidence-based practices.
• Effectively document evaluations, re-evaluations, patient progress, and discharge summaries utilizing advanced software tools for objective measurement of outcomes.
• Develop individualized treatment plans that cater to the specific needs and goals of each patient, considering their physical abilities and age-related requirements.
• Work closely with patients to guide them through their rehabilitation journey, monitoring their progress and adjusting treatment plans as necessary to optimize recovery.
• Adhere to all clinical protocols and policies within the department, ensuring compliance with best practices in patient care.

Patient Population / Client Focus

You will work with a diverse population of patients, primarily focusing on those with orthopedic and neurologic diagnoses. This includes performing post-operative rehabilitation for surgical procedures such as knee replacements and rotator cuff repairs, as well as managing chronic orthopedic conditions. Your role is integral to helping patients regain mobility and improve their quality of life, making your contributions immensely valuable to the local community.

Qualifications

• A Bachelor's degree or higher from an accredited physical therapy program is required.
• Possession of a current Physical Therapist license in the State of Florida; candidates with a temporary permit will also be considered.
• Current BLS/HealthCare Provider certification to ensure readiness in emergency situations.

Compensation and Benefits
