Key Responsibilities\n

• Conduct comprehensive assessments and develop individual treatment plans tailored to the unique needs of each patient, including pre-crisis and crisis plans.

• Complete all required documentation and reporting in compliance with regulatory standards, ensuring timely and accurate submissions.

• Utilize established evidence-based screening tools to inform clinical decision-making, contributing to effective treatment outcomes.

• Collaborate with interdisciplinary teams to create and implement positive behavior support plans that enhance patient care.

• Actively participate in all licensing and accreditation processes to maintain clinic standards and compliance.

• Serve as a vital liaison to various service systems, state agencies, and community partners, advocating for patient needs and program visibility.

• Establish and maintain productive communication channels with relevant stakeholders to enhance community engagement and cooperation.

• Foster partnerships with local, state, federal, and private organizations to bolster program outreach and secure additional resources.

Patient Population / Client Focus\n

You will work primarily with a diverse clientele, including individuals facing emotional health challenges, those recovering from substance use disorders, and other community members with developmental disabilities or those experiencing brain injuries.
