Role: Physical Therapist in Home Health, responsible for assessing patient needs related to mobility, endurance, pain, and medical conditions. Key Responsibilities Evaluate patient functional status and assist in developing therapy plans Provide therapy services within legal scope, following the plan of care, and coordinate with healthcare team Monitor and report patient responses to treatment and condition changes Perform interventions to relieve pain, restore function, and maintain maximum mobility Educate patients, families, and team members on care plans and progress Visit patients, document progress, and develop care plans adhering to state guidelines Report evaluation outcomes, goals, and treatment frequency Adapt to diverse home environments regardless of patient background Qualifications & Skills Current Physical Therapy licensure in practice state Valid driver’s license, vehicle insurance, or access to transportation Preferred CPR certification or ability to obtain within 90 days Compensation: Offers a range based on productivity, including per visit and hourly pay, with benefits such as health coverage, recognition programs, and retirement options. #J-18808-Ljbffr
