Key Responsibilities\n

• Conduct thorough assessments to evaluate clients' social, emotional, and environmental challenges, creating tailored interventions that address their unique needs.

• Develop and implement comprehensive service plans in collaboration with clients, engaging them in setting achievable goals and milestones.

• Provide individual and family counseling to help clients navigate complex issues related to end-of-life care, grief, and adjustment to loss.

• Facilitate crisis interventions and safety planning for clients facing acute challenges to ensure their immediate needs are met.

• Strengthen ties between clients and community resources by coordinating services and facilitating access to benefits and support systems.

• Engage in community outreach efforts to increase awareness of hospice services and promote the agency's mission to serve vulnerable populations.

• Participate actively in case conferences and team meetings to collaborate with colleagues and review client progress.

• Stay informed about best practices in social work, particularly in hospice settings, to enhance service delivery and improve client outcomes.

In this role, you will work with clients who are often navigating profound life changes associated with terminal illnesses and end-of-life concerns. Qualifications\n

• A Master’s degree in Social Work from an accredited program is required.

• Current state licensure as a Licensed Clinical Social Worker (LCSW) is mandatory.
