Key Responsibilities\n

As a Licensed Clinical Social Worker at the American Counseling Center, your responsibilities will include:

• Facilitating educational sessions for patients and families about community resources and post-rehabilitation services to ensure seamless transitions and ongoing support.

• Participating actively in discharge planning, ensuring that patients have a solid plan in place for follow-up care and community reintegration.

Qualifications\n

To be considered for this position, you should have:
