Key Responsibilities\n

• Conduct comprehensive evaluations and assessments of patients and their families to understand their unique needs.

• Actively participate in the development and execution of Individualized Family Service Plans (IFSP) to ensure tailored support.

• Collaborate with interdisciplinary team members to monitor and evaluate patient outcomes, providing vital input and adjustments to care plans as necessary.

• Provide education and direct support to families, empowering them with tools and knowledge that foster a positive environment for their loved ones.

• Adhere to all Federal and State regulations related to social work practice, ensuring compliance in all activities.

• Build rapport and trust with patients and families to create a supportive and understanding atmosphere that enhances the therapeutic relationship.

• Utilize evidence-based strategies within the Natural Learning Environment Practices while delivering care, ensuring a holistic approach to service delivery.

• Facilitate community resources and connections for families navigating the challenges of hospice care.

Patient Population / Client Focus\n

The LCSW will primarily work with patients at our hospice center, focusing on individuals with life-limiting illnesses and their families. We emphasize supporting vulnerable populations who may be experiencing significant emotional distress due to health challenges. As part of our team, you will play a pivotal role in addressing both immediate and long-term emotional and social needs, fostering resilience among families in challenging circumstances.

Qualifications\n

• Master's Degree or Doctorate in Social Work from an accredited institution.

• An active license from the State Department of Public Health; LCSW credential required.
