Key Responsibilities\n

As a Licensed Clinical Professional Counselor at Cooper Care Clinic, your key responsibilities will include:

• Conducting detailed assessments to determine the needs of each patient, allowing for tailored therapeutic approaches.

• Coordinating effective communication amongst supervisors, families, and other relevant parties regarding patient progress and ongoing care.

• Designing a tentative schedule for appointments, home visits, and observations to facilitate patient monitoring and support.

• Implementing and overseeing the structure of in-home observations, ensuring compliance with treatment plans and accuracy in chart reviews.

• Actively participating in Interdisciplinary Team Meetings to share insights and collaborate on patient care strategies.

• Documenting patient progress through comprehensive monthly and quarterly notes, maintaining clarity and transparency in reporting.

• Organizing and facilitating training sessions for staff to ensure competency in executing treatment plans and utilizing documentation tools.

• Providing follow-up care through interviews and home visits to assess the continued needs of clients.
