Key Responsibilities\n

• Conduct detailed assessments to understand each patient's unique needs, ensuring personalized treatment planning.

• Facilitate clear communication with supervisors, direct care staff, family members, and other stakeholders regarding an individual's progress and treatment adjustments.

• Develop and share a structured schedule for appointments, home visits, and supervised observations to monitor advancements in therapy.
\n
• Coordinate and execute home visits to assess in-home environments, perform chart reviews, and conduct staff interviews as necessary.
\n
• Oversee the implementation of patient-centered program plans, ensuring effective delivery and adherence to therapeutic objectives.
\n
• Participate actively in Interdisciplinary Team Meetings to discuss treatment strategies and collaborate on holistic patient care.
\n
• Document monthly and quarterly progress notes to reflect changes in client status and treatment effectiveness.
\n
• Organize and lead training sessions for staff to ensure compliance with program plans and proper documentation procedures.
\n
• Provide ongoing follow-up support, which includes home visits and interviews, to address patients' evolving needs.
\n
• Maintain comprehensive records that are accessible to all relevant parties while respecting confidentiality.
\n
• Act as a liaison between the clinic and the broader mental health community to enhance collaborative care and achieve developmental goals.

• Contribute to the creation and refinement of training materials aimed at improving program effectiveness.

Patient Population / Client Focus\n

In this role, you will have the opportunity to work with a diverse patient population that includes both adults and youth. Your counseling may address various issues including, but not limited to, anxiety, depression, trauma, family dynamics, and substance misuse. Qualifications\n

• A Master’s or Doctoral degree in Counseling, Psychology, or a related field.

• Active and unrestricted licensure as a Professional Counselor (LPC) in the state of Texas.

• A minimum of two years of professional counseling experience, preferably within a clinical environment.

• A strong foundation in family therapy, couples therapy, trauma-informed care, or substance misuse is highly desirable.

• Thorough understanding of relevant legal and ethical standards in counseling practice.
\n
• Excellent interpersonal and communication skills, with the ability to establish rapport with clients from diverse backgrounds.

• Demonstrated ability to collaborate effectively with a multidisciplinary healthcare team.

• Proficiency with Electronic Medical Records (EMR) software and related technologies.

Preferred Experience\n

While the core requirements include essential qualifications, preference will be given to candidates with specialized training or certifications in family or trauma-focused therapy.
