Key Responsibilities\n

• Conduct detailed evaluations and assessments for children under three years of age to identify developmental delays and needs.

• Actively participate in developing and implementing Individualized Family Service Plans (IFSP), ensuring that family goals are clearly defined and achievable.

• Engage with families to monitor progress and outcomes, adapting strategies to meet individual child and family needs.

• Provide education and support to families, equipping them with essential skills to enhance their child's development using evidence-based practices.

• Utilize the coaching model and promote learning within natural environments, tailoring interventions to various learning styles.

• Stay compliant with all pertinent Federal and State regulations governing social work practice.

• Capture relevant data and outcomes to report back on the effectiveness of interventions, ensuring the continuous improvement of services.

Patient Population / Client Focus\n

Your work will primarily focus on infants and toddlers, especially those with disabilities or developmental delays. Through our Children’s Services program, you will play a vital role in supporting young children’s physical, cognitive, social/emotional, and communication skill development. By working closely with families, you will help create a nurturing environment that fosters growth and resilience, helping to bridge gaps in access to services.

Qualifications\n

• A Master’s Degree or Doctorate in Social Work from an accredited institution.

• Current license from the Department of Public Health to practice as a Licensed Clinical Social Worker (LCSW) in California.
