Stretch Synaptic Therapy Services, Inc. is seeking a dedicated and compassionate Physical Therapist to join our team at our Fort Lauderdale, Florida Rehabilitation Center. Key Responsibilities

• Conduct thorough initial evaluations to assess patients' physical capabilities and limitations, utilizing standardized diagnostic tests to inform individualized therapy plans.
• Implement and direct patient treatment protocols that focus on alleviating pain, enhancing mobility, and restoring functionality, while consistently adapting plans based on patient progress and feedback.
• Supervise and mentor Physical Therapy Assistants, ensuring effective execution of treatment plans and adherence to industry best practices.
• Engage in interdisciplinary collaboration during case conferences, contributing insights that help shape comprehensive care strategies for each patient.
• Document patient evaluations, treatment plans, and progress notes accurately in clinical records, complying with all regulatory requirements including OASIS submissions.
• Evaluate necessary home modifications and provide actionable recommendations to enhance patient independence and safety in their living environment.
• Utilize evidence-based practices to administer manual therapy techniques, therapeutic exercises, and advanced modalities such as ultrasound and electrical stimulation.
• Review and update patient care plans periodically, ensuring you're responsive to any changes in the patient's condition or treatment goals.
• Conduct educational sessions with patients and families on rehabilitation techniques, home exercise programs, and appropriate use of assistive devices.
• Maintain timely communication with physicians, case managers, and other healthcare personnel to advance patient care objectives.

Qualifications

• Current, valid physical therapy licensure in Florida, along with active CPR certification.
• Demonstrated proficiency in federal and state regulations regarding physical therapy practice.
