Key Responsibilities\n

• Conduct comprehensive psychosocial assessments of patients and their families to identify strengths, needs, and psychosocial challenges, while also considering their spiritual and cultural values.

• Develop, implement, and periodically revise individualized treatment plans that address identified needs and goals, ensuring they remain aligned with the patient's evolving situation.

• Facilitate educational and therapeutic group sessions for patients, documenting engagement levels, patient goals, and responses to therapeutic activities, which are essential for tracking progress and tailoring interventions.

• Participate in treatment-planning meetings with the multidisciplinary team, providing insights regarding patient progress, challenges, and necessary adjustments to treatment plans.

• Act as a liaison between patients and community resources to enhance aftercare arrangements and ensure continuity of care throughout the hospice experience.

Patient Population / Client Focus\n

You will primarily work with patients facing terminal illnesses and their families within a hospice context. This role requires a deep understanding of the emotional and psychological complexities involved in end-of-life care, as well as the ability to provide empathetic support to patients and their loved ones during these critical times.

Qualifications\n

• A Master’s degree in Social Work from an accredited institution.

• Active licensure as a Clinical Social Worker (LCSW) in California.

• Strong written and verbal communication skills, essential for effective patient interactions and documentation.

• Proven ability to maintain professional boundaries and demonstrate stable and appropriate behaviors with clients.

• Efficient in completing and submitting documentation of services and other required paperwork in a timely manner.

Preferred Experience\n

Compensation and Benefits\n

The salary range for this position is USD 120,000 to 130,000 annually, commensurate with experience and caseload.
