Home Health Physical Therapist to provide patient-centered care in patients' homes across Orange and Osceola Counties. We are seeking a Home Health Physical Therapist to provide patient-centered care in patients' homes across Orange and Osceola Counties. In this autonomous role, you will evaluate patients' functional status, develop and implement personalized treatment plans, and deliver hands-on therapy to improve mobility, strength, and balance. Responsibilities include monitoring progress, adjusting treatments, supporting safe care transitions, and collaborating with interdisciplinary teams. You will supervise Physical Therapist Assistants, educate patients and caregivers, and document care accurately. Qualifications include a graduate degree in Physical Therapy (DPT or Master’s), current PT license, 1-2 years of experience (home health preferred), strong clinical skills, excellent communication, CPR certification, and reliable transportation.
