Key Responsibilities\n

• Conduct comprehensive assessments for patients referred to you in a hospice setting, evaluating their emotional and mental health needs to create personalized, effective treatment plans.

• Engage in active listening and empathetic dialogues to support patients and their families, ensuring they feel valued and understood throughout their care process.

• Develop, implement, and adapt treatment plans in collaboration with interdisciplinary teams, focusing on enhancing patient quality of life and alleviating psychological distress.

• Utilize telehealth platforms to provide therapy sessions, expanding access to care while adapting to patients’ varying needs and circumstances.

• Maintain accurate and up-to-date client records and documentation as per regulatory standards, while regularly evaluating treatment effectiveness and making necessary adjustments.

• Participate in team meetings and case discussions to contribute insights and receive peer support, enhancing the multidisciplinary care approach.

Patient Population / Client Focus\n

Qualifications\n

• A Master’s degree in Social Work (MSW) from an accredited institution.

• A current and valid Licensed Clinical Social Worker (LCSW) license in the state of California is required.

• Minimum of two years of post-graduate clinical experience providing mental health services.
