Key Responsibilities\n

• Conduct thorough assessments of patients’ psychosocial needs to craft tailored treatment plans. This includes evaluating their emotional, social, and practical challenges to ensure comprehensive support.

• Provide short-term counseling, crisis intervention, and case management tailored to individual patient needs. You will engage in therapeutic techniques and interventions that encourage healing and resilience.

• Assist patients in navigating complex health and social systems, including helping them complete forms for entitlements and insurance benefits that empower them to move away from dependence on emergency assistance.

• Facilitate access to prescription cost assistance through pharmaceutical company programs and community resources, ensuring that financial barriers do not hinder patients from receiving necessary medications.

• Act as an advocate for patients in communications with community agencies, ensuring their voices are heard and their needs are met effectively.

• Maintain meticulous patient records and documentation aligned with legal and ethical standards. This includes crafting detailed reports and progress notes that support continuity of care.

• Coordinate transportation assistance for patients when necessary to eliminate barriers to receiving care, demonstrating your commitment to their overall well-being.

• Participate in clinical and team meetings, including case conferences to discuss patient progress and enhance team collaboration.

Patient Population / Client Focus\n

In this role, you will primarily work with a diverse patient population that includes individuals facing a variety of mental health challenges, ranging from anxiety and depression to more complex dual-diagnosis cases. Qualifications\n

• A Master’s degree in Social Work, Psychology, Marriage and Family Therapy, or Counseling is required.

• An active or independent license in one of the following: Licensed Professional Clinical Counselor (LPCC), Licensed Clinical Social Worker (LCSW), or Licensed Marriage and Family Therapist (LMFT).

• Minimum of two (2) years of professional experience in a mental health setting, showcasing your ability to navigate complex challenges and deliver effective therapeutic interventions.

Compensation and Benefits\n

The salary for this position ranges from USD 120,000 to 130,000 per year, commensurate with experience and qualifications.
