AGING AND LONG TERM CARE OF EASTERN WASHINGTONJOB DESCRIPTIONPosition Title: Case Manager IDepartment: Title XIX Case ManagementPosition Type: Full-timeSupervisor: Case Manager SupervisorSupervises: N/AExempt/ Non-Exempt: Non-ExemptPosition Overview:The staff member in this position performs advanced level, specialized case management services for older and/or disabled adults in need of community based long term care services. Positions at this level perform their work and solve problems within the policies and objectives for a specific functional area or program and will devise and set procedures for others. Employees are regularly offered opportunities to socialize together, and volunteer in the community.Benefits:37.5-hour full-time work weekMedical insurance (5 plan options) with low deductibles; premiums are paid 100% for the employeeDental insurance (3 plan options); premiums are paid for the employee and family membersVision care insuranceLong-term disability and life insuranceGenerous paid vacation and sick leave, plus 12 paid holidaysFlexible spending accounts for qualifying medical and dependent care expensesDepartment of Retirement Systems pension/retirement plan457/Roth payroll IRA optionsWellness and employee assistance programPaid premiums for Washington Paid Family & Medical Leave and WA long term care benefitsAmple training for development of program expertise and personal growthWe are a qualifying employer for the Public Student Loan Forgiveness ProgramEssential Functions and Responsibilities include the following:Provides Case Management to vulnerable adults at risk for institutionalization as prescribed by Washington Administrative Codes for the Title XIX program, complying with program regulations and policy directives.Provides comprehensive assessments, using the electronic version of the CARE assessment in the client's home, including completion of service authorizations through ProviderOne.Develops and implements care service plans, coordinating with DSHS staff, the ALTCEW consulting nurse and service providers; maintain and coordinate revisions to the plans. Maintain good working relationships.Provides oversight of the care being provided to the client and ensures paid caregivers receive authorized payment for personal care services, per Washington Administrative Codes.Prepares case file documentation and/or reports.Serves as a mandated reporter of vulnerable adult abuse, neglect, and exploitation. Evaluates client safety concerns and actively coordinates with APS, CPS, and law enforcement on investigations.Provides termination planning and Administrative hearing advisement.Participates in the Administrative Hearing process as needed. May testify as a Department witness and give legal depositions at Administrative Hearings.Provides case management services to facilitate out-of-home placements.Provides supportive functions such as client advocacy, assistance, networking and family support, becoming thoroughly acquainted with community resources available for individuals needing long term care.Maintains client service records, including electronic records, in a complete and timely manner.Other duties may be assigned.Physical DemandsAbility to operate a personal vehicle driving to home visits (within Spokane County) as needed. Demands average 150 to 300 miles per month depending on client needs.Skilled in typing, keyboarding includes up to 7.5 hours per day. Occasional periods of writing.Exhibits manual dexterity for effective operation of vehicle controls and typing.Ability to lift and carry objects up to 20 lbs., including laptops and client files, includes abilities to push, pull, stretch, reach, and maneuver in various directions.Ability to sit, stand and walk, including navigating stairs and uneven terrain, while utilizing bending, stooping, and climbing as needed to access materials.Maintains visual acuity at various distances and operates in diverse environmental conditions, including both indoor and outdoor noisy settings while navigating safely.Ability to speak and listen is adequate for interacting with both employees and the public.Skills, Knowledge and AbilitiesKnowledge of aging and long term care services including the Area Agency on Aging service network. Ability to learn and apply federal and state policy directives and program regulations. Ability to apply observation and analytical techniques in evaluating situations and formulating conclusions. Knowledge of federal, state and local codes and regulations pertaining to community based in-home personal care case management.Proficient with Word and Excel. Ability to perform data entries with speed and accuracy and to quickly learn software platforms that change to support delivery systems. Exhibits compassion for our clients and their well-being.Preferred QualificationsA Master’s degree in behavior or health science and one year paid on-the-job social service experience; or Bachelor’s degree in behavioral or health sciences and two years of paid on-the-job social services experience; or Bachelor’s degree and four years of paid on-the-job social service experience.Minimum QualificationsAt least two years of college level courses in a relevant field and two years of paid on-the-job social service experience; Bilingual or bicultural candidates encouraged to apply.This position requires a criminal background check that qualifies the individual to work with vulnerable adults (WAC 388-133). Must have and maintain a valid driver’s license. Employees driving a privately owned vehicle while on agency business must have liability insurance on the privately owned vehicle. #J-18808-Ljbffr
