Key Responsibilities\n

• Conduct thorough and ongoing assessments to tailor mental health services to the unique needs of each patient, ensuring that care is responsive and relevant.

• Effectively communicate with supervisors, fellow counselors, family members, and other stakeholders to provide updates on an individual’s progress and collaborate on care plans.

• Create and maintain a tentative schedule for appointments, home visits, and other necessary observations to monitor your work advancement and facilitate care delivery.

• Coordinate in-home assessments and offer proper documentation of observations, assessments, and planning during home visits.
\n
• Implement existing program plans, ensuring fidelity in delivery and modification as needed to meet patient goals.
\n
• Engage actively in Interdisciplinary Team Meetings to discuss patient care and contribute insights from your sessions.
\n
• Document monthly and quarterly progress notes that reflect patient developments, adjusting treatment strategies to better serve their needs.
\n
• Assist in developing educational materials and lead training workshops that certify staff competence in program implementation and documentation processes.
\n
• Ensure continuity of care by providing follow-up support through interviews and observations.
\n
• Maintain accurate records to facilitate access to critical information for all stakeholders involved in patient care.

• Act as a liaison between patients, the public, and the mental health community to promote ongoing developmental goals.

• Support the creation of processes and materials aimed at improving our programs and client experiences.

Qualifications\n
