Key Responsibilities\n

• Engage in regular follow-ups with clients to monitor their progress, refine treatment goals, and adjust approaches based on their evolving needs.

• Support the primary care team in identifying patients at risk for mental health crises and work collaboratively to develop actionable prevention plans.

• Maintain accurate clinical documentation of patient interactions and treatment outcomes to ensure high standards of care and compliance with regulatory requirements.

• Educate patients on self-management skills and coping strategies to empower their involvement in recovery and ongoing mental health maintenance.

• Act as a liaison among various healthcare providers to facilitate integrated care, assuring seamless communication and service delivery for patients.

• Regularly update clinic management and team members on patient behavioral health services, guiding evaluations and suggestions for program improvement.

• Promote healthy lifestyle choices among patients with physical disorders, collaborating with them to inspire behavioral changes.

• Maintain adherence to the Code of Ethical Conduct for Clinical Professionals and complete other assigned duties as needed.

Qualifications\n
