Key Responsibilities\n

• Conduct culturally sensitive, strength-based assessments for new patients and their families, taking into account diverse backgrounds and presenting concerns.

• Provide individual and family therapy tailored to the unique needs identified in the assessment process, employing various therapeutic modalities as appropriate.

• Complete required documentation including billing, individualized treatment plans, and insurance authorization forms within designated timeframes to ensure compliance and continuity of care.

• Develop comprehensive crisis plans, safety plans, relapse prevention plans, and wellness plans aimed at maintaining patients’ stable functioning during and after treatment.

• Create effective discharge plans that facilitate a smooth transition for patients as they prepare to complete their treatment.
\n
• Strictly adhere to all federal and state laws, regulatory requirements, and organizational policies to guarantee high standards of care and professionalism.

• Engage in ongoing professional development through training and continuing education opportunities, ensuring that your skills and competencies remain at the forefront of the mental health field.

• Actively participate in case consultations and agency meetings, contributing to team discussions regarding patient care and best practices.

• Complete all licensure requirements and mandatory training to maintain compliance with professional standards.

Patient Population / Client Focus\n

As a Licensed Mental Health Counselor at Concentra, you will primarily work with diverse outpatient clients experiencing a range of mental health challenges. This includes, but is not limited to, anxiety, depression, relationship issues, trauma, and more. Qualifications\n

• A Master’s degree in social work, counseling, marriage and family therapy, or a related field that qualifies for licensure as a mental health therapist.

• Must hold licensure as a Mental Health Therapist (LPC, LMFT, or LCSW) in the State of Arizona.

Preferred Experience\n

Prior experience in outpatient mental health settings is preferred. Familiarity with evidence-based practices, crisis intervention strategies, and integrated treatment methodologies will enhance your effectiveness in this role.
