Employee Wellness Case Manager & Intake Specialist
Endeavors Inc.
Endeavors seeks a Case Manager/Intake Coordinator to assess eligibility and enroll CBP employees and eligible dependents into the CBP Workforce Wellness Program. You will conduct intake interviews, verify documents, provide referrals, and develop individualized wellness plans in collaboration with clients. Working with a multidisciplinary team, you will coordinate service delivery, support follow-up, and advocate for client access to wellness services while maintaining confidential records and #J-18808-Ljbffr Endeavors Inc.
