Work Environment

This full-time position is an onsite role, primarily serving patients within the home setting. Key Responsibilities

• Direct physical therapy treatment plans aimed at preventing disabilities, relieving pain, restoring muscle function, and maintaining optimal performance for patients, all in accordance with their individualized care plans.
• Evaluate clients' functional abilities through rigorous diagnostic assessments involving muscle, nerve, and joint functionality, leading to precise treatment strategies.
• Oversee the implementation of therapy plans carried out by Physical Therapy Assistants, ensuring adherence to established care protocols.
• Conduct thorough assessments of patients' home environments and provide recommendations to facilitate rehabilitation and enhance their independence.
• Administer manual and therapeutic exercises, guide patients in non-manual physical activities, and provide education on their treatment programs.
• Prepare and maintain comprehensive clinical records, including evaluations, care plans, progress notes, and discharge summaries, ensuring compliance with regulatory requirements.
• Engage actively in interdisciplinary team meetings to evaluate and adapt treatment plans in response to patient progress and feedback.
• Provide consultation and education to patients and their families, empowering them through knowledge and skills to enhance their care experience.
• Utilize EMR software, particularly Web PT, for accurate and timely documentation of all patient interactions and therapy progress.
• Continuously assess and revise therapy plans based on patients' evolving needs, ensuring the highest quality of care.

Patient Population / Client Focus

Our clients include individuals of all ages requiring home health services, including post-operative rehabilitation, chronic pain management, and mobility enhancement. You will work with patients on a one-on-one basis, allowing you to customize treatments and establish meaningful relationships that foster trust and encourage compliance with therapy regimens.

Qualifications

• Completion of a Doctorate in Physical Therapy (DPT) program.
• Active Physical Therapist license in New Jersey, with a clean record devoid of sanctions or board proceedings.
• Demonstrated proficiency in various manual therapy modalities and physical rehabilitation techniques.
• Expertise in formulating and executing successful therapy plans that align with evidence-based practices.
• Proficient in utilizing electronic medical records systems, particularly Web PT, for efficient documentation.

Preferred Experience

• Strong background in home healthcare or similar settings will be advantageous.
• Experience in mentoring or supervising Physical Therapy Assistants in clinical settings.
• Knowledge of community resources available for patient support and enhancement of therapy outcomes.

Compensation and Benefits

The annual salary for this position ranges from USD 100,000 to 120,000, commensurate with experience, plus a comprehensive benefits package designed to support the well-being of our full-time team members.
