Work Environment

Located onsite in our hospital facility, you will work alongside a diverse team of healthcare professionals in a dynamic environment that emphasizes teamwork and community engagement. The work schedule generally spans from 9:00 a.m. to 6:00 p.m., with occasional evening clinics until 8:00 p.m. Participation in community outreach events or health fairs may also be required on weekends, ensuring our services extend beyond the clinic's walls.

Key Responsibilities

• Conduct thorough health assessments and screenings for patients of all ages to identify medical conditions and develop individualized care plans.
• Diagnose and manage both acute and chronic health issues, providing preventive care and health promotion tailored to the community's needs.
• Collaborate with community organizations to design and implement health education programs that foster awareness and engagement among residents.
• Deliver culturally sensitive care, advocating for patients' rights to ensure they have access to necessary healthcare services.
• Coordinate referrals and ensure continuity of care, following up with patients to address ongoing health concerns effectively.
• Document patient encounters accurately in compliance with regulatory standards, ensuring that all medical records are maintained properly for continuity and quality care.

Patient Population / Client Focus

You will primarily serve a diverse patient base, including underserved populations who may face barriers to accessing care. Qualifications

• Current certification as a Nurse Practitioner
• Active state nursing license

Compensation and Benefits
