Work Environment

This position is based onsite at our Hyattsville hospital facility, and you will have the opportunity to interact with a multidisciplinary team of healthcare providers. You will typically work weekdays from 8:00 a.m. to 5:00 p.m., with occasional evening clinics or weekend outreach events to promote health education in the community.

Key Responsibilities

As a Family Nurse Practitioner at Belares’ Family Care Center, you will:

• Conduct thorough health assessments, obtaining comprehensive patient histories and performing physical examinations for individuals and families.
• Diagnose and manage a broad range of acute and chronic health conditions, devising individualized treatment plans that prioritize preventive care and health promotion.
• Utilize diagnostic tools effectively by ordering and interpreting necessary tests and imaging, ensuring timely and accurate patient diagnosis and care.
• Educate patients and their families about health maintenance, disease prevention, and self-care strategies, fostering a culture of informed health choices.
• Collaborate with local organizations and community leaders to design and deliver impactful health education programs that address prevalent health concerns in the area.
• Advocate for patients' rights and access to care, ensuring culturally competent services are delivered with respect and empathy.
• Maintain meticulous medical records in adherence to regulatory standards, upholding patient confidentiality and maximizing the efficiency of care transitions.
• Participate actively in community events and health fairs, promoting wellness initiatives and providing screenings to enhance community engagement.
• Stay abreast of the latest developments in community health and evidence-based practices to continuously improve patient care.

Patient Population / Client Focus

Your primary focus will be on serving diverse populations throughout Hyattsville and surrounding areas. This includes patients from various socioeconomic backgrounds, including those with limited access to healthcare resources. Qualifications

• Current certification as a Nurse Practitioner and an active state nursing license.
• A Master’s degree in Nursing from an accredited institution is required.
• A minimum of 3-5 years of clinical experience is preferred, particularly in community health or public health settings.
• Demonstrated knowledge of health assessment procedures and practices, particularly for family health.

Compensation and Benefits
