Key Responsibilities

• Conduct thorough primary care services including initial health assessments, routine check-ups, and comprehensive evaluations to ensure a complete understanding of patient needs.
• Perform onsite health risk assessments, identifying potential health issues and developing tailored intervention plans that improve health outcomes.
• Provide extensive patient education regarding wellness strategies, preventive measures, and making informed healthy lifestyle choices, empowering patients to take charge of their health.
• Assist and guide patients in managing chronic conditions, ensuring appropriate treatment plans are established and maintained for effective disease management.
• Oversee and coordinate care delivered by the center's medical staff, ensuring seamless communication and quality management across the board.
• Participate in health education initiatives, wellness events, and community fairs to foster health awareness and promote preventative measures among community members.
• Collaborate closely with staff members, fostering a team-oriented environment focused on enhancing patient engagement and overall health outcomes.

Patient Population / Client Focus

The Family Nurse Practitioner will primarily interact with a diverse patient population, including children, adults, and seniors. Qualifications

Compensation and Benefits
