Key Responsibilities

As a Nurse Practitioner in our cardiology department, you will:

• Conduct thorough assessments and diagnose both acute and chronic cardiovascular conditions with a focus on personalized care.
• Develop comprehensive treatment plans tailored to each patient's needs, integrating evidence-based practices and patient preferences.
• Order and interpret diagnostic tests, including EKGs and echocardiograms, to provide timely and accurate evaluations.
• Prescribe appropriate medications and therapies, monitoring patient responses and making adjustments as necessary.
• Provide education to patients and their families regarding heart health, disease prevention strategies, and lifestyle modifications.
• Collaborate with various healthcare professionals to ensure a seamless continuum of care, always putting the patient at the forefront.
• Maintain meticulous medical records utilizing our electronic health record system, ensuring compliance with state regulations and organizational policies.
• Participate in quality improvement initiatives aimed at enhancing patient outcomes and enhancing service delivery.
• Stay current with the latest developments in cardiology and healthcare practices to provide the most relevant information and treatment options to patients.

Patient Population / Client Focus

This role primarily focuses on geriatric patients residing in an assisted living facility. Many patients will have complex health care needs, including multiple comorbidities related to cardiovascular health. Your expertise will be instrumental in managing their care effectively, aiding in both preventative measures and necessary interventions.

Qualifications

To be considered for this position, applicants must possess:

• A Master's or Doctoral degree in Nursing from an accredited program, with a specialization in Cardiology.
• Active Nurse Practitioner licensure in the state of Maryland.
• Certification in a recognized specialty area as a Nurse Practitioner.
• A minimum of 2 years of clinical experience in cardiology or a related field.
• Strong clinical assessment and diagnostic skills.
• Outstanding communication and interpersonal skills, essential for interacting with patients and their families.
• A collaborative approach to working in a fast-paced healthcare environment.

Preferred Experience

We highly value candidates with additional experience in chronic disease management, especially related to hypertension, heart failure, and other cardiovascular issues, as well as those familiar with healthcare protocols related to assisted living environments.

Compensation and Benefits

Health Plus Child and Family Center offers a competitive salary range of $120,000 to $140,000 per year, reflecting the candidate's experience and qualifications.
