Work Environment

This role is conducted onsite in our state-of-the-art medical facility located in Hyattsville, Maryland. Key Responsibilities

• Conduct comprehensive assessments, including history-taking and physical examinations, to diagnose and manage a wide range of acute and chronic health conditions.
• Develop, implement, and evaluate individualized care plans based on patient needs, incorporating evidence-based practices and clinical guidelines.
• Provide patient education on preventive health measures and chronic disease management, ensuring patients and their families are informed about treatment options and lifestyle modifications.
• Order, interpret, and evaluate diagnostic tests and laboratory results to guide treatment decisions.
• Collaborate effectively with physicians and other healthcare staff to ensure continuity of care and timely referrals to specialists when necessary.
• Document all patient interactions accurately within our electronic health record (EHR) system, ensuring compliance with legal and ethical standards in healthcare documentation.

You will primarily be serving adult patients with various health concerns, emphasizing both preventive care and the management of chronic illnesses. Qualifications

• Master's degree in Nursing with a specialization in Family Nurse Practitioner (FNP) or equivalent from an accredited institution.
• Current and valid Nurse Practitioner license in the State of Maryland, with national certification from a recognized certifying body.
• Strong clinical reasoning and decision-making skills, with the ability to work independently and collaboratively within a healthcare team.
• Effective communication skills to educate and interact with patients, their families, and interdisciplinary teams.
• Demonstrated commitment to continuous professional development and evidence-based practice.

Preferred Experience

• At least two years of clinical experience as a Nurse Practitioner in an internal medicine or primary care setting.
• Experience with electronic health records (EHR) and telemedicine platforms; familiarity with the specific software used within the clinic is a plus.
• Acute Care Nurse Practitioner certifications will not be considered for this role.
