Bloom Health Medical Clinic is actively seeking a dedicated Internal Medicine Nurse Practitioner to join our clinical team onsite in Bowie, Maryland. Work Environment

This role is based onsite, allowing the Nurse Practitioner to engage directly with patients in a home care setting. The work hours are conveniently scheduled from Monday to Friday, 9 am to 5 pm, providing a balanced work-life commitment. Key Responsibilities

• Conduct comprehensive assessments and formulate diagnostic impressions for patients, working under the supervision of the attending physicians.
• Provide individualized care that addresses acute, chronic, and long-term health issues, including diagnostic testing and therapeutic interventions.
• Effectively implement disease prevention measures and health promotion tactics tailored to the needs of our diverse patient population.
• Educate patients and their families about health management, preventive care strategies, and responsible medication management to promote informed decision-making.
• Maintain accurate patient records and documentation in compliance with clinic policies and regulatory standards, ensuring continuity of care and thorough follow-up.
• Collaborate with healthcare teams, utilizing an interdependent approach that encourages consultation and referrals when necessary.

Patient Population / Client Focus

The ideal candidate will engage with a varied demographic that includes individuals with both complex chronic illnesses and acute health conditions. Our patient population may include elderly patients, those managing multiple health issues, as well as families needing comprehensive preventive care. As a Nurse Practitioner in this role, you will have the opportunity to make significant positive impacts in patients' lives by implementing tailored care plans that consider unique medical histories and lifestyle factors.

Qualifications

• Must be a graduate of an accredited Nurse Practitioner program.
• Possession of an active state license or eligibility to practice as a Certified Nurse Practitioner (CNP) in Maryland, alongside national certification.
• Certification as a Family Nurse Practitioner (FNP) from an approved certifying organization.
• Acute Care Nurse Practitioners will not be considered for this position.

Preferred Experience

We require a minimum of 2 years of relevant experience in internal medicine or a related field. Ideal candidates should demonstrate critical thinking abilities and strong clinical judgment in managing a diverse patient population. Familiarity with electronic health records (EHR) systems will be advantageous to facilitate efficient patient management and documentation.

Compensation and Benefits
