Work Environment

This position is performed onsite at our outpatient office located in the heart of Jessup, Maryland. In this structured yet collaborative environment, you will work alongside a diverse team of healthcare professionals to provide comprehensive care. Regular hours are Monday through Friday, from 9:00 a.m. to 6:00 p.m., with occasional evening shifts extending until 8:00 p.m. You may also partake in weekend community outreach events or health fairs, supporting our mission outside traditional office hours.

Key Responsibilities

• Conduct meticulous health assessments, utilizing evidence-based methods to evaluate patient conditions.
• Develop individualized care plans addressing both acute and chronic medical conditions, emphasizing preventive care strategies, such as immunizations and screenings.
• Collaborate closely with local community organizations to design, implement, and promote health education initiatives that empower community members.
• Deliver culturally competent care, ensuring that all patients are recognized and respected, focusing on health equity and access to services.
• Coordinating with specialists and other healthcare providers, facilitate timely patient referrals and follow-ups to ensure continuity of care.

Qualifications

• Possession of an active Nurse Practitioner certification and a valid state nursing license.
