Key Responsibilities

• Conduct thorough patient assessments, including detailed medical history reviews and physical examinations to evaluate the health status of residents.
• Diagnose and manage acute and chronic conditions, ensuring timely interventions that align with the healthcare needs of each resident.
• Develop and execute individualized treatment plans that may include prescribing medications, therapies, or referrals to specialists as needed.
• Provide preventive care and educate residents on wellness strategies and lifestyle modification to enhance their quality of life.
• Collaborate effectively with physicians, nurses, and other healthcare professionals to coordinate comprehensive patient care and management.
• Maintain accurate electronic health records (EHR), documenting clinical findings, treatment plans, and follow-up care in compliance with best practices.

Patient Population / Client Focus

The primary patient population served in this role consists of elderly residents within the assisted living facility who may have various health challenges and requirements. As a Nurse Practitioner, you will focus on geriatric care, managing chronic diseases such as diabetes, hypertension, and cognitive impairments, and addressing the comprehensive health needs of this vulnerable population with empathy and understanding.

Qualifications

• A Master of Science in Nursing (MSN) or a Doctor of Nursing Practice (DNP) from an accredited institution is required.
• An active and unrestricted Nurse Practitioner license in Maryland, or eligibility for licensure in the state is necessary.
• Board certification as a Family Nurse Practitioner (FNP), Adult-Gerontology Nurse Practitioner (AGNP), or a related specialty is essential.
• DEA registration and the ability to prescribe medications in Maryland.
• Proficiency in using electronic health record systems, with the capability to quickly and accurately document patient information.
• Excellent clinical assessment, diagnostic reasoning, and interpersonal communication skills for effective patient interactions.

Preferred Experience

• A minimum of 2 years of experience in primary care, internal medicine, or a specialty area relevant to geriatric care is strongly preferred.
• Experience in chronic disease management, including knowledge of care protocols for conditions such as diabetes and hypertension.
• Demonstrated ability to work collaboratively within a multidisciplinary healthcare team, prioritizing patient-centered care.
• Strong background in patient education and health counseling, helping residents understand their health conditions and treatment options.
