Work Environment

This position is based onsite at our home care facility, where you will work Monday through Friday from 9:00 AM to 5:00 PM. The home care setting allows for a personalized approach to patient care, enabling both flexibility and the ability to engage directly with patients in their own environments. Our facility is equipped with modern medical technology, ensuring you have the necessary tools to deliver high-quality cardiovascular care.

Key Responsibilities

• Conduct comprehensive evaluations and assessments for 16-20 cardiology patients daily, ensuring thorough documentation of patient histories and clinical findings.
• Develop and implement individualized care plans, conducting follow-up appointments to monitor patient progress and manage ongoing treatment strategies.
• Perform diagnostic stress tests, interpreting results to provide actionable insights into patients' cardiovascular health.
• Collaborate with a multidisciplinary team, including physicians and allied health professionals, to optimize chronic disease management and enhance patient outcomes.
• Educate patients and their families on lifestyle modifications, medication adherence, and self-management strategies pertinent to cardiovascular health.
• Participate in quality improvement initiatives aimed at elevating patient care standards and practice protocols.

Patient Population / Client Focus

The Nurse Practitioner will primarily serve adult patients suffering from various cardiovascular conditions, including hypertension, arrhythmias, congestive heart failure, and coronary artery disease. Your clinical expertise will help in the management of complex medical histories and multifactorial health challenges, ensuring tailored care that meets the unique needs of each individual.

Qualifications

• A Master’s degree in Nursing from an accredited program.
• Board Certification as a Family Nurse Practitioner (FNP) or Adult Nurse Practitioner (ANP).
• Current DEA license and state nursing licensure.
• A minimum of 2 years of experience as a Nurse Practitioner, preferably in a cardiology or similar setting.

Preferred Experience

• Hands-on experience in conducting and interpreting diagnostic tests relevant to cardiology.
• Proficiency in chronic disease management and patient education strategies.
• Familiarity with electronic health record (EHR) systems to document patient encounters and care plans efficiently.

Compensation and Benefits

The salary for this full-time Nurse Practitioner role ranges from USD 120,000 to 140,000 per year, complemented by a comprehensive benefits package.
