Work Environment

This role is based onsite at our Assisted Living Facility, where you will work closely with a diverse team of healthcare professionals to ensure that residents receive personalized and compassionate care. The work schedule is typically Monday through Friday from 9:00 a.m. to 6:00 p.m., with some evenings and weekends required for community outreach programs and health fairs. An average weekly commitment of approximately 40 hours is expected, providing opportunities to engage with residents and their families regularly.

Key Responsibilities

• Conduct thorough health assessments and screenings for assisted living residents, focusing on their unique medical and emotional needs.
• Diagnose and manage both acute and chronic medical conditions, emphasizing preventive care strategies that enhance residents’ quality of life.
• Collaborate with interdisciplinary teams and local organizations to design and deliver relevant health education programs tailored to the community.
• Ensure that all care provided is culturally sensitive, advocating for resident rights and navigating healthcare access challenges.
• Facilitate patient referrals as necessary, coordinating follow-up care to ensure optimal health outcomes for residents.

Patient Population / Client Focus

As a Nurse Practitioner within our Assisted Living Facility, you will primarily serve elderly residents who require ongoing medical attention, often facing complex health issues. Qualifications

• A valid Nurse Practitioner certification and an active state nursing license are mandatory.
