Responsibilities
- Coordinate and deliver comprehensive end-of-life care to terminally ill patients in alignment with hospice philosophy and regulatory guidelines.
- Serve as primary case manager for an assigned caseload (typically 12–18 patients), conducting assessments, developing and updating individualized care plans with the interdisciplinary team.
- Provide skilled nursing interventions, including medication management, pain and symptom control, wound care, and patient/family education on disease process and hospice care.
- Offer emotional support and crisis intervention to patients and families; educate on medications, symptom expectations, and care expectations.
- Maintain timely and accurate EMR documentation; ensure compliance with Medicare, Medicaid, and state/federal hospice regulations; participate in interdisciplinary team meetings.
- Participate in on-call rotation and respond promptly to after-hours patient needs.

Requirements
- Current Registered Nurse license in California
- CPR/BLS certification
- Minimum two years of recent field experience; hospice experience preferred; travel candidates must have hospice experience
- Valid driver’s license, current auto insurance, reliable vehicle, and eligibility to drive
- BSN preferred; five years hospice nursing experience preferred
- Ability to work in a hospice/acute care setting and collaborate with a multidisciplinary team
