QMHP - Youth Crisis Outreach Team Specialist

Overview The Youth Crisis Outreach Team (YCOT) Specialist works under the direction of the YCOT Supervisor. The role involves providing crisis intervention, screening, referral, relapse prevention, and case monitoring services to youth and families experiencing crisis situations within LifePath Systems catchment area. The Specialist plays a vital role in implementing the YCOT program, offering immediate support to youth along with their caregivers and families experiencing crisis situations. Responsibilities Develop and maintain professional contact with youth, families, and partner providers for medical, social, educational, psychiatric, and related services to navigate crisis resolution. Provide on‑site and off‑site screening assessments with rapid crisis intervention and crisis management to youth and families experiencing behavioral or emotional crises with potential life‑threatening behavior or acute psychiatric crisis. Deliver trauma‑informed interventions and strategies to de‑escalate youth in psychiatric crisis. Collaborate with the Mobile Crisis Outreach Team, Crisis Intervention Services, and Crisis Intervention Team to facilitate stabilization in the least restrictive environment. Coordinate and provide appropriate referral and linkage services under the guidance of the YCOT supervisor or designee, and develop and implement relapse‑prevention and safety‑planning strategies. Ensure ongoing communication with individuals served and their families to provide support and guidance. Collaborate with Emergency Department, Department of Family and Protective Services, or local agencies to divert from inpatient and law‑enforcement interventions. Ensure a seamless connection and coordination with community mental‑health resources and clinics for continued support beyond the crisis period. Coordinate, plan, and implement the Safety Plan as required for appropriate levels of care. Seek clinical consultation as needed from the YCOT Supervisor or designated supervisor. Provide accurate and timely reporting metrics to the YCOT supervisor. Make accurate and effective assessments and recommendations regarding youth’s mental health needs in compliance with rights and preferences, and all relevant state and federal statutes, as well as LifePath Systems policies and procedures. Develop and maintain rapport with persons in crisis using modalities such as Motivational Interviewing and other relevant methods to communicate and relate with callers to resolve crises. Travel & Vehicle Requirements Travel is required and can vary from 20% to 100% per day within LPS catchment area. The role may require use of a fleet vehicle and/or a personal vehicle for program needs, including field and home visits, off‑site meetings, co‑located sites, and pick‑up of items for program needs. If transport of individuals is required, it will only be allowed with a company vehicle and approval from management. Shift & Schedule Hours and shift assignments: Position requires a flexible schedule due to rotating shifts, including evenings. Hiring for shifts MON‑FRI 12:00 PM–8:00 PM. Hours may vary; flexibility is needed. Performance Targets Complete assessments as soon as possible and within the required response time based on urgency. Ensure documentation is completed concurrently and signed by the end of shift. Follow‑ups completed at 24 hours, up to 90 days if clinically necessary, but no less than 30 days. Shift report communicated at the end of each shift. Ninety percent of full chart reviews receive a score of 80 or higher. Qualifications Bachelor’s degree in psychology, social work, or a related behavioral health field. Minimum of two years of experience in community mental health working with youth and families having severe persistent mental illness and substance use disorders, including crisis services. Master’s degree in human services field preferred. English/Spanish bilingual preferred. Must have reliable transportation and a valid driver’s license. Experience in providing psychosocial education, skills training, or assessments, particularly with the target population. Experience participating in a treatment team that develops and monitors treatment plans for persons with severe persistent mental illness and substance use disorders. Knowledge, Skills, & Abilities Think clearly and logically, and use de‑escalation techniques in potentially dangerous or emotional situations. Knowledge of trauma‑informed care principles and strategies. Knowledge of community resources, case‑management principles, objectives, standards, and methods; knowledge of program policies and procedures. Write clearly and professionally on all services and internal communication. Respond to individuals with patience, empathy, and understanding. Work under general supervision with moderate latitude for initiative and independent judgment. Work independently and collaboratively, communicating effectively with supervisory staff and community partners. Organized and detail‑oriented; computer proficient; able to prioritize appropriately. Microsoft Office Suite skills – Outlook, Word, Excel, PowerPoint, etc.; familiar with company‑specific programs and software. Experience in working case‑management with youth and family or field crisis response teams. Assess individuals’ needs, coordinate services, communicate effectively, and train others.
