Description

• Conduct evaluations for patients in both inpatient and outpatient settings, crafting customized treatment plans tailored to individual needs
• Implement physical therapy interventions aimed at enhancing mobility, strength, balance, and overall function
• Collaborate with a multidisciplinary team including nurses, physicians, and case managers to achieve optimal patient outcomes
• Educate patients and their families on therapeutic goals, discharge protocols, and at-home exercise regimens
• Accurately document evaluations, treatments, and patient progress in adherence with facility and regulatory protocols
• Participate actively in care team rounds and interdisciplinary discussions to ensure cohesive and coordinated care

Education

• Doctorate in Physical Therapy (DPT) or an equivalent degree obtained from an accredited institution

Certifications

• Valid state licensure as a Physical Therapist
• CPR certification is highly preferred
• Specialty certifications (e.g., neurological, geriatric) are advantageous but not mandatory

Skills

• Previous experience in a clinical setting is strongly preferred
