This position is entirely remote, offering you the flexibility to manage your responsibilities from your desired location while still being an integral part of our Social Work Services team. Although primarily remote, occasional visits to the community health clinic may be necessary for crisis management and collaboration with multidisciplinary teams. Key Responsibilities\n

• Independently manage a designated caseload of patients, utilizing a comprehensive approach to evaluate and assess social, emotional, financial, and cultural factors that impact their treatment plans.

• Advocate for patients by empowering them to take control of their treatment decisions, ensuring they feel supported and heard throughout their healthcare journey.

• Lead healthcare team coordination in addressing issues surrounding protective services, life care decisions, and advance directives for patients facing complex psychosocial challenges.

• Provide crisis intervention services to patients and their families, contributing to an effective response to high-stress situations.

• Collaborate closely with case managers and healthcare professionals to ensure the psychosocial needs of patients are addressed, enhancing overall treatment and discharge plans.

• Deliver education and resources to both patients and families, coordinating with community agencies to facilitate seamless access to required services.

• Supervise and mentor junior social work staff, interns, and volunteers, fostering a culture of continuous learning and professional development within the team.

• Participate in specialty team rounds and patient care conferences to share insights, strategies, and updates related to case loads.

• Respond to psychosocial needs swiftly, ensuring no avoidable delays in treatment plans or discharge processes.

• Be available for rotational on-call support to address urgent needs that may arise within the medical center.

Compensation and Benefits\n

The salary range for this position is competitive, with an annual compensation of USD 120,000 to 130,000.
