Key Responsibilities\n

• Collaborate closely with medical and mental health professionals to develop and implement coordinated care plans that address the physical and psychological aspects of patient health.

• Coordinate post-discharge services with community organizations to ensure continuity of care, including connecting patients with essential resources and support systems.

• Provide educational resources to patients and families regarding treatment options, mental health resources, and ways to navigate their healthcare journey effectively.

• Participate actively in case management discussions, hospital committees, and quality improvement initiatives, leveraging your social work expertise to enhance organizational practices.

• Develop and implement comprehensive discharge plans, ensuring that all patients’ needs are met upon leaving the clinic.

Qualifications\n

Compensation and Benefits\n
