Work Environment

The position is located onsite at our hospital facility in Seneca Falls, NY, where you will be an integral part of our collaborative care team. Our multidisciplinary team is composed of physicians, nurse practitioners, physician assistants, medical assistants, and support staff, including registered nurses and care managers. This collaborative environment ensures that our Nurse Practitioners can focus on providing exceptional primary care, care coordination, and managing the overall health of our patient population.

Key Responsibilities

• Provide exceptional primary healthcare services to patients during office visits for both routine and acute health issues.
• Conduct annual wellness visits and health risk assessments with an emphasis on thorough documentation and patient education.
• Coordinate care with other healthcare providers, specialists, testing facilities, and community agencies to ensure patients receive comprehensive, quality care.
• Lead population health initiatives by ensuring eligible patients receive necessary screenings and follow-ups, such as biennial evidence-based breast cancer screenings.
• Support the care team in phone triage, patient outreach, and managing patient inquiries effectively.
• Educate patients on their diagnosed conditions, treatment plans, and preventive health measures to empower them in their care journey.
• Occasionally conduct home visits when deemed necessary to assess patient needs in their home environment.
• Participate in promotional activities to enhance community engagement and awareness of health services provided.
• Report directly to the Medical Director and collaborate closely with operational leadership.

Patient Population / Client Focus

Your primary focus will be on adults, particularly older adults on Medicare, in our community. By utilizing an innovative model of care that emphasizes quality over volume, you will have a direct impact on keeping patients healthy and engaged in their health care journey, ensuring they can live fulfilling lives without frequent hospital visits.

Qualifications

• A Master of Science in Nursing, Doctor of Nursing Practice, or recent completion of an NP Certificate Program.
• National certification as a Family, Adult, or Adult-Gerontology Primary Care Nurse Practitioner through AANP or ANCC.
• Active and valid Nurse Practitioner license in New York, non-probationary.
• Active DEA license for prescribing medications.
• Proven experience in primary care settings such as internal medicine, geriatrics, or family medicine.

Preferred Experience

• Familiarity with HCC documentation, ICD-10 coding, and Health Risk Assessments.
• Ability to work collaboratively within a multidisciplinary team to achieve optimal patient outcomes.
• An eagerness to engage with innovative healthcare models that prioritize population health.
• Bilingual proficiency in relevant languages to serve our diverse patient population is a plus.

Compensation and Benefits

The annual salary for this position ranges from $115,000 to $150,000, commensurate with experience and expertise. We also offer a competitive benefits package, including:
