Key Responsibilities

• Conduct comprehensive initial patient assessments, including histories and physical examinations, to determine individual health needs.
• Maintain accurate and up-to-date patient records, ensuring all interactions are logged correctly at each appointment.
• Interpret diagnostic tests and results, providing tailored recommendations for follow-up appointments and treatment options based on findings.
• Deliver high-quality care while adhering to organizational standards and productivity metrics.
• Act as a primary resource for patients navigating their ongoing care needs, providing consistent counseling and support—especially for those managing chronic or critical conditions.
• Teach patients about health maintenance strategies, disease prevention methods, and evidence-based treatment plans, emphasizing harm reduction practices.

Patient Population / Client Focus

Qualifications
