Work Environment

This role is primarily onsite, allowing you to engage deeply with patients within their own environment, enhancing the quality and effectiveness of care. Working a standard schedule from 9:00 a.m. to 6:00 p.m. with occasional later shifts, including evenings and weekends, your work will often involve community health programs, health fairs, and outreach events aimed at addressing the health disparities faced by underserved populations.

Key Responsibilities

• Conduct thorough health assessments and screenings, tailoring your approach to fit the needs of each individual or family in the community.
• Diagnose and manage both acute and chronic medical conditions, utilizing evidence-based practices for preventive care and health promotion.
• Develop and implement community health education programs in collaboration with local organizations and stakeholders, focusing on the health literacy of our population.
• Advocate for equitable access to healthcare services, ensuring that all patients receive culturally sensitive care.
• Coordinate referrals and follow-up care to maintain continuity in treatment, enhancing patient outcomes and satisfaction.
• Maintain accurate, detailed medical records to comply with regulatory standards and for the continuity of care.
• Participate actively in community outreach events and health fairs to raise awareness and provide essential health screenings.
• Stay informed on health trends, evidence-based practices, and regulatory policies affecting community health.

Patient Population / Client Focus

As a Nurse Practitioner at NextGen NP Solutions, you will primarily serve a diverse patient population, including families and individuals with varying healthcare needs. You will have the opportunity to make a significant impact on the health of those in your community by addressing not just physical health needs but also social determinants of health.

Qualifications

• Master's degree in Nursing or Doctoral degree from an accredited program.
• Current New York State Nurse Practitioner (APRN) license, with DEA and NPI numbers required.
• Certification as a Family Nurse Practitioner (FNP), Adult-Gerontology Nurse Practitioner (ANP), or Adult-Gerontology Primary Care Nurse Practitioner (AGNP) by the American Nurses Credentialing Center (ANCC) or the American Academy of Nurse Practitioners (AANP).
• Proficient in using electronic health record (EHR) systems to document patient encounters and manage care effectively.

Preferred Experience

• 3-5 years of clinical experience, preferably in primary care or community health settings.
• Strong clinical assessment, diagnostic skills, and experience in managing chronic diseases.
• Exceptional communication and interpersonal skills, capable of working effectively with a diverse population.
• Ability to work both independently as well as collaboratively within a healthcare team.

Compensation and Benefits
